Daily Lessons from Life 25 April 2016 - Tragic SMRT Lapses ...

"SMRT concludes investigation into accident that led to death of two staff members - The ST 25 Apr 2016

SINGAPORE - Critical safety procedures were not followed when the accident which led to the death of two SMRT maintenance staff members happened, SMRT said on Monday (April 25) after completing its investigations into the mishap near Pasir Ris MRT Station on March 22.

The investigations found that the deaths of Nasrulhudin Najumudin, 26, and Muhammad Asyraf Ahmad Buhari, 24, could have been avoided had safety measures been put into place.

Before a work team is allowed onto the track, protection measures must be taken. This includes setting the speed limit on the affected track sector to 0 kmh so that no train can enter on automated mode, and deploying watchmen to look out for approaching trains."

A sad situation as nobody wanted the fatal accident to happen. Yet, 2 young men lost their lives and their family members emotionally impacted. It is certain that whoever had been the 'mistakes' in executing the safety protocols could not be feeling any better.

Lessons for me are:

1. accidental death due to overlooking established safety procedures/protocols is the most difficult to accept death. No one win in this situation. The 'people who made that fatal mistake' will be inconsolable and hopefully not having the guilt for a life time;

2. while it is very hard to forgive the people whose mistake took away the lives of the 2 young men, holding on to the hurt will not be helpful. Somehow, the surviving members of the family and friends have to find some form of closure. If justice is perceived to had been served for the punishment by those responsible for the lapses, so be it. If somehow, someone can find the heart to forgive and move on, that will be extraordinary'

3. still, if someone has to let off steams and hurts, let they come out in full force and all at once. Only after the vending and releasing the stress, hurts and tension can THAT someone begin to look for healing. Time is the best healer. Yet, only when the mind accepts that 'it had happened and nothing one do could ever bring the dead back to life', will healing start.

A sad situation with no winner. RIP to the dead and be strong to the surviving family members, colleagues and friends.
